In compliance with Nevada statutes and A.V.M.A. guidelines, a written referral for canine physical therapy services (except wellness/sports performance) is required from your treating veterinarian. To facilitate this process, there is a prescription/referral form on my website or contact us and we will be happy to send one to your veterinarian. He/she can email it back to us or you can bring it to your initial visit. After the initial evaluation, your veterinarian will receive a report detailing the objective findings, assessment, and treatment plan for your dog's rehabilitation services. Your veterinarian will also receive written and/or verbal progress reports throughout the animal's treatment. A referral is not required for wellness service, which can aid in pet mobility improvement.
If your pet is ill (vomiting, diarrhea, respiratory symptoms, etc.), please reschedule your canine physical therapy appointment and consult your vet. We want to ensure the best outcomes for your dog's rehabilitation services.
There is a $50 fee for cancellations less than 24 hours in advance. Emergencies are understood, but my schedule for dog rehabilitation services is usually set a week ahead. I'll do my best to accommodate changes for your pet's mobility improvement.
